
TBH offers the following benefits:
Health Insurance
TBH has PPO health insurance coverage through Blue Shield of California (Effective date 12-01-06) Although Blue Shield is our primary coverage; the company also offers insurance through Kaiser which is a $20 co-pay plan. (Please see attached information sheets for coverage specifics).
Dental
TBH's dental benefits are through Delta Dental. There is a $50 Copay per year which is waived for diagnostic and preventive services. The Calendar year maximum per patient is $1,000. There is no charge for diagnostic and preventive (x-rays, exams, consultations and cleanings). Basic services (restoration, oral surgery, endodontics, periodontis and sealants) are covered at 80% with crowns, jackets, cast restorations and prosthodontics (some limitations on prosthodontics) are covered at 50%. This plan does not offer orthodontics. NOTE: DELTA DENTAL DOES NOT OFFER AN “OPEN ENROLLMENT” PERIOD AND WILL NOT ALLOW ELIGIBLE EMPLOYEES OR DEPENDENTS TO COME ONTO THE PLAN AFTER THE INTIAL ENROLLMENT. INITIAL ENROLLMENT IS A ONE TIME OFFER ONLY!
Disability Policy
TBH pays for a disability policy through Standard Insurance Company. The LTD benefit is 60% of the first $10,000 for your monthly pre-disability earnings reduced by deductible income. The maximum is $6,000 before reduction by deductible income and the minimum is $100.00. The benefit waiting period is 90 days. The maximum benefit period is determined by your age when the disability begins. PLEASE NOTE THAT INSURANCE BENEFITS ARE AVAILABLE TO ALL EMPLOYESS THE FIRST OF THE MONTH FOLLOWING DATE OF EMPLOYMENT.
Fitness Club Membership
TBH will pay for your membership at 24 Hour Fitness. This is an "All Club" membership (includes "Basic" and "Sports" Clubs but excludes the "Sporting" clubs).
401(k) Plan
TBH's 401(k) plan is administered by ExpertPlan.com. The benefit is $0.50 on the $1.00 to 6% of your gross providing you are contributing 6% of your gross. If you are only contributing 3% of your gross TBH will match $0.50 on the $1.00 to 3% of your gross while if you are contributing 15% of your gross TBH will match $0.50 on the $1.00 to 6% of your gross. Each employee has control over their investments through ExpertPlan.com's web site. There are about 15 plans to disburse contributions into ranging from conservative to aggressive. 401(k) participation begins the first quarter (Jan 1st, April 1st, July 1st or Oct 1st) after date of employment. (Example: Employment date is January 1st, entry date to the 401(k) plan is April 1st)
Family Members
Family members can be added onto the health/dental and 24 Hour Fitness membership at your expense through payroll deductions.
Paid Time Off and Continuing Medical Education
PTO/CME is on an accrual basis, accrued per pay period worked. Any un-used vacation at termination is paid out. Any un-used CME is NOT paid out at termination.

TBH has twelve (12) paid holidays per year (see list).
Call & Moonlighting
Packages can be augmented with weekend call and moonlighting. A doctor who works one weekend call per month in Northern California and 50 instead of 40 hours per week can make in the neighborhood of $260,000 inclusive of benefits.

California rural community positions (such as the position in Eureka) also exist and typically have reimbursement packages up to 20% higher than standard basic salaries. Rural positions are generally harder to fill and so assignments typically command better base salaries and more attractive benefit packages. The secondary advantage of these locations is that they are coastal with a significantly lower cost of living than urban areas.
Rural positions also have four rather than three weeks vacation, 2 personal days, 12 sick days per year, 5 family death benefit days, $3,200 cash benefit allowance, and well over $20,000 in on-call reimbursement per year, 1 week continuing education with $500 stipend, life insurance $20,000 paid with option to purchase up to $300,000, three 5% merit step increases and 5% increase at 10 year anniversary. PERS retirement and deferred compensation are also available in some locations.

Part-time positions and weekend call opportunities to supplement private practice or other professional activities are also available at TBH.
